HOW TO APPLY:

• The $200 application fee is not due until your application is approved!
• Review items needed list below
• Go to your trip page.
• Locate the trip you are going on
• Click the “Trip Application ” button
• Create a Managed Missions login and password
• NOTE: Save this login information for later use

INFORMATION YOU NEED TO COMPLETE YOUR APPLICATION:

• You will need to supply your Christian testimony and answer why you want to be a part of the team
• Passport Number (if you do not have a passport, please contact the Global Department)
• Names & contact info for 2 people from Church of the King who know you
• Names & contact info for 2 emergency contacts (close family or parents)
• Minors need both parents contact information (emails & cell)
• Insurance information
• Primary Care Doctor name and contact information
• List of current medications and allergies to food or medicines
